Consolidated Waiver
Community Living Waiver
The Community Living Waiver (CLW) in Pennsylvania is a Medicaid Home and Community-Based Services (HCBS) waiver program. It is designed to support individuals with physical disabilities who require a level of care that would typically be provided in a nursing facility but who choose to receive that care in their own homes or communities. The CLW provides funding for a range of services aimed at helping participants live as independently as possible while remaining integrated in their communities.
Person Family/Family Directed Support Waiver
The Person/Family Directed Support (P/FDS) Waiver is a Medicaid Home and Community-Based Services (HCBS) waiver program in Pennsylvania. It provides funding and support services to individuals with intellectual disabilities or autism, allowing them to live in their homes or communities rather than in institutional settings. The P/FDS Waiver emphasizes individual and family direction in planning and managing services, offering participants greater control over their supports.
Under the P/FDS Waiver, individuals and their families can develop a person-centered plan tailored to their unique needs and preferences. They have the flexibility to choose and manage their service providers, including hiring and training their own direct support professionals. This approach aims to promote self-determination, independence, and community inclusion for waiver participants.
Aging Waiver
Home and Community-Based Services Waiver for Individuals Aged 60 and Older. Aging Home and Community-Based Waiver Services may be available to Pennsylvanians over the age of 60 to enable them to continue to live in their homes and communities with support and services.
A Home and Community-Based Services Waiver for Individuals with Traumatic Brain Injury (TBI). If you have a medically determined diagnosis of traumatic brain injury (TBI), the COMMCARE Waiver can help you remain as independent as possible.
If you are an adult with a severe physical disability, the Independence Waiver may be able to help you live or remain in the community and remain as independent as possible.
The OBRA Waiver is a Home and Community Based Waiver program that may help you if you have a developmental physical disability to allow you to live in the community and remain as independent as possible.
If you have a physical disability, the Attendant Care Waiver and state funded Act 150 program may be available to you to continue to live in your home and community with support and services.
